- Confirm correct side

Check Wilson frame
- arm board testing, even with bed
- highest N possible, nearby
- tightened with arm pads
- C clamp with bar going proximal, mid thigh similar to TLIF
- C-clamp ON SAME SIDE for LESS
- Positioning, silver portion of arm board is north, arm board with foam right away AS HIGH as possible
- 4 pillows on bed, 3 for under feet, one for under knees
- Wilson crank counter clockwise
- Check steps

Prep stand
- Wilson crank
- Shaver with blade and tape
- Adhesive prep mitts x2
- Alcohol
- 4x4 bucket
- 10-10s x4
- Chloraprep orange x2
- Upper body bear hugger

C-clamp
- Same side as surgeon
- Interlaminar: bend of knee
- Transforaminal: mid-thigh
- 2 white towels folded and on either side of bed for armpits
- SCDs
- +/- foley
- Set up lights (elbows away from surgeon, 1 over anesthesia, 2 north, 3 south) – see right sided case

Bed
- Allen table with Wilson frame
- Wilson frame commonly only able to handle pt <300 lbs, at UCSD, have 500 lb frame
- Foam arm pads x2 (shorter part more caudal)
- Horseshoe face mask
- Silver arm board clamps x2, as proximal as possible locked on bed
- Align distal arm board w/proximal edge of Wilson frame
- Pillows x 4
- Green donut for head/face (now square as of 07/23/24)
- All cephalad equipment north of patient’s wrists
- Check equipment and pedals are plugged in
- Fluoro shields x2

- Depomedrol: 40mg per level

- 2cc of 0.5% methyl blue (Provay 5mg/mL) + 9cc omnipaque 300mg/mL
